Any additional information you can provide will be important. SRM has reported this as an adverse event where death or serious injury was likely to occur as a result of this failure. during use in the field this tourniquet experienced a failure where the windlass broke and could no longer be retained in place. A second tourniquet was applied and the patient was able to be evacuated successfully.
